Kindergarten Forms
Due to new laws regarding patient privacy, Waukesha School District no longer has access to mailing lists based on birth dates. If you have a child that will be 5 years old by September 1, 2011, please contact our school (262-970-1700) as soon as possible for an information packet. Please share this information with your friends and neighbors!

Due to copyright laws we are unable to offer a copy of the Child Development Review but a form can be requested by calling Heyer school office at 262-970-1700.
Please bring the Student's Immunization Records to Registration. They are required to complete state mandated form.
